    Welcome!

    New patients appointments are released on a regular basis.

    If you cannot see an appointment available, please check the calendar regularly.

    Once you are able to request your new patient appointment:

    1) You’ll be registered with our electronic health record system (CHARM)

    2) You’ll receive new patient forms emailed to you

    3) Please complete your forms at least one week prior to your appointment

    First office visits are extended visits. Please allow up to

    75 minutes.

    You must first be an established patient (aka have had a new patient appointment) to schedule an IV push or intramuscular injection.

    If you need to reschedule or cancel your appointment, please send a message to us via the charm patient portal at least 24 hours prior to your appointment time.

    Return visits may be considered "extended visits," depending on complexity and length required at the time of the visit. Please allow 60 minutes for return visits.

    Our providers are in network with most Regence and Premera insurance plans however it is the patient’s responsibility to verify eligibility prior to the appointments.

    A superbill for office visits will be provided upon request for you to submit to your insurance to seek reimbursement.
